Raul Rosas Jr.’s Facial Condition Explained: What Is Pallister-W Syndrome?

This weekend, Raul Rosas Jr. will be making UFC history, but the attention to his unique appearance garners as much curiosity as his dynamic fighting style.

On Saturday, Sept. 26, the 21-year-old bantamweight will headline the UFC Fight Night against the veteran Raoni Barcelos at the Meta APEX in Las Vegas. This will mark Rosas Jr.’s debut as a main event fighter and also the youngest fighter ever to headline a UFC event.

Rosas, who is currently ranked 12th, has a 12-1 record, which includes a unanimous-decision win over former title challenger Rob Font at UFC 326.

The quick rise in popularity that Rosas Jr. has experienced has exposed him to a much wider audience, with many new fans looking for an explanation of his unique facial features. Many media outlets have linked this to a rare genetic condition called Pallister-W syndrome.

However, neither Rosas nor the UFC has spoken out about the condition to the media.

What is Pallister-W syndrome?

Pallister-W syndrome, which is also known as W syndrome, is an extremely rare congenital condition. According to the National Institutes of Health, the number of cases is less than one in every million and the pattern of inheritance is described as X-linked recessive. The Genetic and Rare Diseases Information Center at the National Institutes of Health likewise refers to it as a disorder resulting from a genetic change and notes that the symptoms usually appear either in infancy or at birth.

The syndrome may result in a set of identifiable facial features, and medical descriptions note a broad forehead, the eyes being widely set apart, the opening of the eyelids sloping downwards, a broad and rather flat nasal bridge, abnormalities affecting the upper lip or the palate, the absence of the upper central incisors and a prominent jaw.

Other reported symptoms may be seizures, skeletal abnormalities, differences in growth and neurological or developmental problems. It doesn’t mean that every person linked with the condition shows all of the symptoms listed, and it would be inappropriate to deduce any specific neurological or cognitive feature about Rosas Jr. merely from his appearance.

Even in the medical literature, the condition is considered to be extremely rare; one of the early accounts mentioned only two brothers who had it, and a later case report included another patient who showed similar features. Pallister-W syndrome is still regarded as an ultra-rare disorder in modern medical databases.

Several reports have also wrongly linked Rosas Jr.’s medical condition with the special permission he required in order to compete before he turned 18. Reports from 2022 at the time show that the Nevada Athletic Commission review was necessary due to his age, and when Rosas was only 17 years old, he took part in Dana White’s Contender Series and therefore needed both parental paperwork and special approval from the commission.

Fans And Some Fighters Have Longed Asked About Rosas’s Appearance

Since Rosas Jr. entered the UFC, his face has frequently come up among MMA fans, at times sparking real curiosity and, unfortunately, at other times attracting mockery.

Several Reddit threads raised questions as to why Rosas Jr. looks different from his brother. Some users referred to Pallister-W syndrome, while others said they knew very little about the condition. As before, similar debates have surfaced on MMA discussion boards over the years, with some spectators wrongly believing his facial features resulted from damage from fighting.

Even fighters have remarked on his looks. Sean O’Malley made a public joke regarding Rosas Jr.’s appearance following the teenager’s UFC debut in 2022.

In contrast, Rosas Jr. has mostly based his public image on the events that take place inside the Octagon rather than on comments regarding his appearance. At the age of 21 he is already 6-1 in the UFC and is now facing his most important fight yet against Barcelos.
